NDA - A foundation that helps diagnose rare diseases
I designed an intuitive, user-friendly platform for a foundation specializing in rare disease diagnostics. The goal was to enhance accessibility, improve user experience, and streamline the diagnostic process for patients and medical professionals.

Project overview
I was responsible for redesigning the foundation’s website to enhance clarity, usability, and engagement. One of the biggest challenges was the diagnostic form—its length and complexity led to high drop-off rates, preventing users from completing it. My goal was to create a streamlined, user-friendly experience that would make the diagnostic process easier and more accessible while improving completion rates.
Timeline
The project took 3 months, covering research, wireframing, prototyping, and redesigning both the website and diagnostic form.
Background
The foundation’s existing website and form had a major usability issue - users found the form overwhelming, often abandoning it halfway. Given the importance of early diagnosis in rare diseases, improving this experience was critical. My goal was to restructure the form and overall website layout to make the process as clear and efficient as possible, while still gathering the necessary medical information.
Process
I took a structured approach to redesign the platform, addressing usability challenges and ensuring a seamless experience.
Research & Planning
Before jumping into design, I analyzed data provided by the client to understand where users were struggling. The findings showed that the long, uninterrupted form was the biggest issue—users felt overwhelmed and uncertain about how much time they needed to complete it. Based on this, I outlined key areas for improvement, including form length, structure, and visual clarity.
Design & Prototyping
With clear goals in place, I created wireframes and interactive prototypes to test different solutions. I introduced a two-step approach: a short, initial screening form with just 10 questions to assess risk, followed by the main form for more in-depth data collection. To reduce cognitive overload, I divided the long form into collapsible sections, making it easier for users to navigate. A progress bar was also introduced, giving users a clear visual indicator of how far they had come and how much was left.
Implementation
After finalizing the prototypes, I worked on implementing the design with usability and accessibility in mind. The new form and website were designed to be clean, structured, and easy to navigate, ensuring that users could complete the process without frustration. Additionally, I introduced a fixed help box that provided users with an option to contact support if they needed assistance. This ensured that those struggling with the form could still get the help they needed without abandoning it.
Solution
The final design significantly improved the website’s usability, ensuring that users could complete the diagnostic form with ease while staying engaged throughout the process.
Two-Step Diagnostic Form
Pre-Screening Form – A short, 10-question form that quickly assesses the user’s risk level and encourages them to proceed to the full form if needed.
Redesigned Main Form – A streamlined, structured form broken into collapsible sections, allowing users to focus on one part at a time without feeling overwhelmed.
Enhanced User Guidance & Experience
Progress Bar – A dynamic progress tracker that updates in real time, showing users exactly where they are in the process and how much is left.
Collapsible Sections – Instead of facing a long, never-ending list of questions, users now see only relevant sections, making the form feel shorter and more manageable.
Fixed Help Box – A persistent help section offering live support via chat or phone, ensuring that users who need assistance can get help without frustration.
Personalization
Customizable settings allow users to tailor scheduling preferences and priorities to their unique needs.
